## About
Digbi Healths Chronic Illness Management is based on a precision biology approach. Every Member that we work with gets an at home genetic test, at home gut microbiome test, and continuous glucose monitor. Digbi leverages that information to identify the biological root cause of the members condition and how they will react to different treatment paths. From there, individuals work with their care team to treat across comorbidities and conditions members may have, creating one entree point for members with cardiometabolic and inflammatory conditions. Focusing on nutritional changes to the meals they are currently eating based on their biology and illness to identify small changes that are sustainable for the member and will create large impact to their health. The goal of the program is to identify the driving factors, educate the individual rather than standardized care, and identify small changes we can make to drive health outcomes on a sustainable level.
