The leading eCKM care platform for whole health
Comprehensive early cardio-kidney-metabolic (eCKM) care that treats hypertension, hyperlipidemia, obesity, and early kidney disease.
Description
FlyteHealth delivers a fully integrated clinical intervention and prescribing program for early cardio-kidney-metabolic (eCKM) care. Our focus on whole health, combined with the use of an EMR to support clinical care integration, reduces care fragmentation. Our programs delivered durable health outcomes and independently validated ROI in 2025 and 2026.
The FlyteHealth Clinical Approach
Impacting total cost of care (TCOC) and improving measurable clinical outcomes require actively treating the costliest chronic conditions in patients living with multiple comorbidities. FlyteHealth focuses on adherence, team-based care, and the use of proven branded and non-branded medications. Board-certified clinicians prescribe, treat, and track patients, with care available seven days a week. FlyteHealth combines multidisciplinary medical management with intensive lifestyle modification, cellular-connected remote monitoring, and annual biometric laboratory monitoring.

The FlyteHealth Care Model: Prevent, Treat, Maintain eCKM
All members begin through a single portal, complete biometric and HRA data, including SDOH and PHQ-8, and are acuity- and ASCVD-scored utilizing our proprietary AI algorithm. Patients are then routed into personalized care pathways:
- Prevent eCKM: Intensive, CDC/AHA-aligned lifestyle interventions, virtual appointments and personalized education, annual biometric tracking and care plans to prevent disease progression
- Treat eCKM: Comprehensive medical management for diabetes, hypertension, hyperlipidemia, early kidney disease and obesity, delivered by in-house board-certified internists, obesity specialists, endocrinologists, nurses, nurse practitioners, medical assistants, dietitians, patient coordinators, community health workers and health coaches.
- Maintain eCKM: Continuous clinical oversight, care team follow-up, and biometric monitoring designed to lock in progress and prevent disease regression.
Validated Cost Savings
Independent 2026 Segal actuarial data demonstrate $313 PMPM in medical savings for high-acuity cohorts and approximately $100 PMPM in average total medical savings within one year, along with $29.7 million in first-year pharmacy cost avoidance and less than half the medical cost trend growth compared with matched controls (12.3% vs. 26.1%). A 2025 Milliman evaluation further confirms $3.6 million in early pharmacy cost avoidance among pilot populations.

Features
In-house Dedicated Care Teams & Continuous support
FlyteHealth’s multidisciplinary care team, including American Board of Obesity Medicine (ABOM)-certified physicians, nurse practitioners, registered dietitians, and care coordinators, provides coordinated, personalized care to help patients prevent, treat, and manage early cardio-kidney-metabolic (eCKM) conditions, including obesity, prediabetes, type 2 diabetes, hypertension, and early kidney disease.
Through the FlyteHealth app, patients can track nutrition and activity, access educational resources, schedule appointments, and communicate regularly with their care team. In-house dietitians and coaches provide ongoing lifestyle and behavioral support, helping patients stay engaged and achieve lasting improvements in their health.
Labs, HRA, & Medical History
Our clinically proven care model goes beyond traditional weight-loss programs by combining personalized lifestyle support with responsible prescribing of GLP-1s and lower-cost obesity medications when clinically appropriate. This integrated approach helps patients manage their weight alongside related chronic conditions. Before the first medical appointment, each patient completes baseline labs and a comprehensive health risk assessment that includes social drivers of health and the PHQ-8. These insights help the care team develop a personalized treatment plan that addresses the patient’s medical needs, behavioral health, and lifestyle.
Personalized Lifestyle Support, Medication & Connected Monitoring
We combine personalized lifestyle support with clinically appropriate prescribing of GLP-1 and non-GLP-1 anti-obesity medications, guided by a patented algorithm developed from more than 30 years of obesity medicine expertise.
Connected devices enable our care teams to monitor progress and adjust treatment as patients’ needs evolve. Beyond obesity, our integrated model addresses related conditions such as prediabetes, high cholesterol, and hypertension. Once patients achieve their weight-loss goals, our maintenance program provides ongoing clinical and lifestyle support to help them sustain their progress and improve their long-term health.
