Pack Health offers digital health coaching to improve outcomes across more than 25 chronic conditions. The approach extends support to people with chronic conditions who understand what they should be doing to manage their health, but who can’t be successful on their own, often due to inherent barriers. Pack Health is designed to move the needle on the metrics that directly affect your bottom line through personalized, one-on-one, evidence-based support on a HIPAA-secure digital coaching platform. Pack Health serves as a complement to clinical care provision, supporting individuals and driving behavior change at home, where the majority of health management truly occurs.

With each individual we engage, Pack Health addresses chronic conditions, behavioral health, and social determinants of health. Through a weekly cadence of goal-setting, our Health Advisors learn about the individual’s motivators and barriers. The relationship-based coaching and goal-centered approach enhances member self-efficacy resulting in improvements in key clinical indicators, such as blood glucose, and behavioral factors, such as diet, exercise and medication adherence.
